Dengue Virus
According to World Health Organization, there are 50 million cases of Dengue infection worldwide every year. The infection is an epidemic in over 100 countries, especially in subtropical regions. The virus belongs to the avivirus group and is transmitted by Aceder mosquitoes.
The primary and secondary infections of dengue virus exhibit dierent clinical proles. Primary Dengue infection is the most common type of dengue illness. It is associated with mild to high fever, head- ache, muscle pain, and skin rash. Secondary infection is known as Dengue Hemorrhagic Fever (DHF) or Dengue Shock Syndrome, and often results in high fever and, in many cases, hemorrhagic events and circula-
tory failure. The fatality rate in patients with Dengue Shock Syndrome can be as high as 44%.
Diagnosis
Serological studies demonstrate that in primary Dengue viral infections, IgM antibodies are detect-able by the 5th day of infection and remains detectable for 30 – 60 days. Whereas IgG antibodies appear by the 14th day of infection and persist for life.
In contrast, during secondary infections, specic IgG and IgM levels signicantly increase by day 1- 2 and day 20 after infection, respectively. Thus, dierent proles of humoral immune responses in primary and secondary dengue viral infections can be used for dierential diagnosis, which is of clinical signicance in predicting progression and prognosis of the disease.
